Payment is expected at the time of service. We accept cash, checks, Visa, Mastercard and Discover.
The doctors and other health care practitioners at The Paracelsus Clinic will provide you with appropriate (HICF 15000) form for all bills when requested. These are suitable for submission to all US insurance companies. None of the practitioners, nor Dr. Thomas Dorman are preferred providers in any system. Dr. Thomas Dorman is NOT a Medicare provider and does not have a UPIN number. Medicare patients are welcome at the clinic. However, none of the services provided at, or ordered by, the practitioners at the clinic are Medicare benefits. American patients, who are beneficiaries of Medicare, may, under the 1998 Kennedy Kassebaum law, contract with Dr. Thomas Dorman or other practitioners at the clinic privately.
If this applies to you please print and sign the Patient's Request and Consent for Non-Medicare Services form and bring it with you to your first appointment.
